Twenty minute consultation via phone or video conferencing: free

Diagnostic assessment: $225

One hour therapy session: $175

Insurance: Accepting Blue Cross and Blue Shield of Minnesota and Blue Plus (Blue Cross), Health Partners, Cigna, Aetna, Medica, Ucare, Medical Assistance (MA), Medicare, and OPTUM insurance (including Medica, Surest, Bind, UMR, United Health Care, United Behavioral Health, Mayo Medica, UHC PMAPS, Medica PMAPS).

If you are private pay, you have the option to submit your bill to your insurance as an out of network claim.

If you submit your bill to your insurance as an out of network claim, I can NOT guarantee coverage for any service provided, but I will help you in providing any necessary documentation. Coverage will be determined by your individual insurance plan. Full payment will be required at the time of services and reimbursement for services will be between you and your insurance company.

If you are on a MN Care plan, you will not have out-of-network (OON) benefits. Mn state law prohibits private payment from anyone on MNCare plans, Prepaid Medical Assistance Program (PMAP), and State Medical Assistance plans. If you are on a MNCare plan that I’m not credentialed with, I will not be able to see you for services at this time.

Medical providers are required to give a Good Faith Estimate (GFE) specifying anticipated fees prior to the start of services. You will receive a GFE of therapy based on self-pay rates.
